To whom it may concern, I am writing about the decision to refuse my application for credit. I request the following in writing within the statutory period, or within 30 days if none applies. 1. Whether an automated, algorithmic or artificial intelligence system made, recommended or materially influenced this decision, and the name of the system or vendor. 2. The specific reasons for the decision, in plain language, and the principal factors that led to it. 3. The categories and sources of data about me that were used, and a copy of that data. 4. How I can correct or add to that data, and whether the decision will be reconsidered if I do. 5. Whether a person with the authority to change the decision has reviewed it, and how I can request such a review. 6. How I can contest the decision, including any deadline and any external body I can turn to. Under the Equal Credit Opportunity Act I am entitled to the specific principal reasons for this decision. From 1 January 2027 Colorado law also requires notice of automated decision-making technology, a plain-language explanation, the ability to correct my data and a request for human review. Please treat this letter as a formal request and, where applicable, as a request for access to my personal data and for human review. I reserve all rights.
